Pre-Entry Planning
- Identify and Classify Confined Space: Determine if the space meets the definition of a confined space (e.g., limited entry/exit, not designed for continuous occupancy).
- Conduct Hazard Assessment: Identify potential hazards (e.g., hazardous atmospheres, electrical hazards, oxygen deficiency).
- Develop Safety Plan: Establish procedures for entry, rescue, and emergency response.
- Establish Atmospheric Monitoring: Continuously monitor air quality for hazardous gases (e.g., CO, H2S), oxygen levels, and flammable vapors.
Entry Procedures
- Permit Required Confined Spaces: Obtain a written permit before entering a confined space.
- Entry Team: Assemble a team of trained personnel, including an entrant, an attendant, and a rescue team.
- Establish Communication: Ensure clear and reliable communication between the entrant, attendant, and rescue team.
- Wear Personal Protective Equipment (PPE): Utilize appropriate PPE (e.g., respirator, safety harness, protective clothing).
- Use Ventilation and Barricading: Ensure adequate ventilation and barricade the entry point to prevent unauthorized entry.
During Entry
- Monitor Atmosphere: Continuously monitor atmospheric conditions and evacuate if necessary.
- Stay Alert: Entrants should remain alert to potential hazards and communicate any concerns.
- Stay Within Designated Area: Confine work activities to the designated area and avoid wandering.
- Use Tools Safely: Ensure tools are well-maintained and used correctly.
Exit Procedures
- Safe Exit Plan: Establish a safe exit plan before entering the confined space.
- Emergency Equipment Available: Keep emergency equipment (e.g., stretcher, lifting devices) readily accessible.
- Notify Attendant: Inform the attendant when exiting the confined space.
Rescue Procedures
- Trained Rescue Team: Designate a rescue team and provide training in confined space rescue techniques.
- Rescue Equipment Available: Keep rescue equipment (e.g., tripod, retrieval system) readily accessible.
- Summon Help Immediately: If an emergency occurs, summon help immediately and follow the established rescue plan.
Additional Safety Measures
- Training and Certification: Ensure all personnel involved in confined space work are adequately trained and certified.
- Hot Work Permit: Obtain a permit for any hot work (e.g., welding, cutting) in a confined space.
- Confined Space Log: Maintain a log of all confined space entries, including personnel involved, hazards identified, and corrective actions taken.
- Regular Inspections: Conduct regular inspections of confined spaces to identify potential hazards and maintain safe conditions.
