Mrx with complain of Abdominal pain for two days duration Part Two:-HPI Mr x a 50 yrs. old male patient comes to ST Marry hospital with complain of persistent sharp abdominal pain. Additionally he has loss of appetits, nausea and vomiting for 02 days duration in all quadrants, generalized wenkness and dizziness. He has no known family history of disease. He has no major past medical history including dyspepsia. Discussion question #1 List the patient's problem Define your hypothesis and explain the niechanism causing the problem. Part three: - P/E General Appearance: Acute sick looking V/S: T: 36.7oc AT, RR: 19 B/M, PR: 88 BAM, BP: 80/50 mmHg HEENT: pink conjunctiva, sunken eye, dry oral mucosa Chest: He has regular breathing pattern with clear chest.Sland $2 are welt heard, symmetric chest movement, no gallop, no murmur. Abdomen: He has slightly distended abdomen with active bowel sound. He has tympani sound in all four quadrants. Has tenderness during deep palpation. No Organomegally GUS: no any redness of urine, dysuria, testicular tenderness, no sexual abnormality Integumentary. No skin pustule, no redness, bluish discoloration on palm. Poor skin turgor. MSS: generalized body weakness. 1
